Help with House Plant

    I have a house plant that is over 30 years old. I don't know it's name or how to properly care for it.

    It doesn't actually have leaves, it has little segments that grow from the tips. Each segment is about 3/4 inch long. It seems to go in cycles of looking good and bad. When it looks good, sometimes it will have some small yellow flowers grow at the tips. I would describe this plant as draping.

    How do I care for this plant?
